Frequent manipulation of central lines (drawing blood, infusing medications) is the strongest risk factor for CRBSI. Each access event increases the chance of contamination. Continuous feeding rather than bolus can also reduce aspiration risk in high-risk patients, but posture remains the single most impactful, universally applicable measure. Additional strategies include oral care, subglottic suctioning when available, prokinetic agents (metoclopramide/erythromycin) for feed intolerance, and considering post-pyloric feeding in patients with persistent high aspiration risk. Fiber addition helps bowel regularity and microbiome health but does not specifically mitigate aspiration. Consistent head-of-bed elevation should be part of standard EN safety bundles.

GI fistula output contains bicarbonate; loss leads to metabolic acidosis. In PN, acetate salts (Na acetate, K acetate) are metabolized to bicarbonate and correct acidosis. Adding chloride worsens acidosis. Lipid and protein adjustments do not address acid-base imbalance. Individualized PN design helps restore metabolic balance in high-output fistula patients.

Vitamin D deficiency in PN patients leads to osteopenia and osteomalacia, manifesting as bone pain and fractures. Vitamin K deficiency causes coagulopathy, vitamin A deficiency leads to night blindness, and vitamin E deficiency causes hemolysis. Fat-soluble vitamins must be included in PN to prevent complications.
