Vitamin A deficiency impairs rhodopsin regeneration in the retina, leading to night blindness as an early symptom. Severe deficiency can progress to xerophthalmia, keratomalacia, and blindness. Patients on long-term malabsorptive states or PN without adequate supplementation are at risk. Vitamin E deficiency causes neuropathy and hemolysis; vitamin K deficiency causes coagulopathy; vitamin D deficiency leads to rickets/osteomalacia. Vitamin A supplementation in PN (~3300 IU/day in adults) prevents deficiency. Clinical suspicion should arise in PN patients with ocular symptoms.

Autonomy gives competent patients the right to refuse or discontinue medical treatment. The clinician must respect the choice, ensuring informed decision-making and offering comfort care. Continuing PN against the patient’s will is unethical. Family or legal intervention is unnecessary when patient autonomy is clear.
