When the GI tract is obstructed and EN is not feasible, central PN is indicated. Peripheral PN cannot provide adequate long-term support due to osmolarity limits. Copper deficiency is associated with anemia, neutropenia, and neurologic myelopathy; selenium deficiency with cardiomyopathy and skeletal muscle weakness; chromium deficiency with insulin-resistant hyperglycemia and peripheral neuropathy. In PN, zinc is typically dosed around 2.5–5 mg/day in stable adults, with higher doses in patients with significant gastrointestinal losses (e.g., additional 12 mg/L of small-bowel drainage or ostomy output). Correction involves adding adequate zinc to PN and addressing losses; clinical signs often improve over weeks. Monitoring should include trace element panels and inflammatory markers to properly interpret levels.

Peripheral PN can be used for short-term support (<7–10 days) if the osmolarity is kept below ~900 mOsm/L to avoid phlebitis. Central PN (PICC, tunneled, port) is required for hyperosmolar solutions and longer durations. Ports and tunneled catheters are intended for long-term therapy. Peripheral PN provides partial caloric/protein support, not full nutrition, and is best for temporary use when central access is not available.
