In placental abruption, maternal hemorrhage and coagulopathy can rapidly compromise both mother and fetus. The priority is maternal resuscitation—establishing two large-bore IVs, initiating balanced resuscitation with crystalloid judiciously, and ensuring blood products are available or pre-notified at the receiving facility. Restoration of maternal perfusion improves uteroplacental blood flow and fetal oxygenation. Routine tocolysis is contraindicated in abruption due to risk of masking bleeding and worsening outcomes. Cushing’s triad reflects rising ICP and impending herniation. Early recognition allows temporizing measures: elevate head of bed, optimize sedation, maintain normocapnia, and consider hyperventilation if herniation imminent. Sepsis causes hypotension and tachycardia. Autoregulation is protective, not pathologic. MI causes chest pain, not this triad. Prompt recognition and intervention in flight can preserve cerebral perfusion until neurosurgical care is available.

Norepinephrine is the preferred first-line vasopressor in cardiogenic shock because it raises MAP predominantly via alpha-adrenergic vasoconstriction with limited increase in heart rate. Dopamine has more arrhythmic complications and worse outcomes. Epinephrine increases lactate, tachyarrhythmias, and myocardial oxygen demand, used as second-line or adjunct. Phenylephrine reduces heart rate but can decrease cardiac output by excessive vasoconstriction. During transport, norepinephrine provides reliable titratable support while awaiting definitive reperfusion or mechanical support.

Early antiplatelet therapy with aspirin is a cornerstone of STEMI care and is one of the most impactful mortality-reducing prehospital interventions prior to reperfusion. Chewed, non–enteric-coated aspirin inhibits cyclooxygenase-1, reducing thromboxane A₂–mediated platelet aggregation and limiting thrombus propagation in the culprit coronary artery. Routine high-flow oxygen for all MI patients is no longer recommended; instead, oxygen is titrated to maintain SpO₂ ≥ 94% because hyperoxia can cause coronary vasoconstriction and increase oxidative stress without benefit in normoxemic patients. Prophylactic lidocaine has not demonstrated mortality benefit and may cause proarrhythmia. Morphine may relieve pain and reduce sympathetic drive but can mask worsening ischemia, cause hypotension, and possibly interfere with antiplatelet absorption; thus it is not a mortality-reducing first-line step. In the air medical environment, giving aspirin promptly, maintaining hemodynamic stability, and expediting definitive reperfusion remain priorities to improve outcomes during transport.

Targeted temperature management (32–36°C for 24 hours) improves neurologic recovery post-cardiac arrest in comatose patients. Cooling can begin prehospital using cold IV fluids or surface devices. Hyperventilation worsens cerebral ischemia by reducing perfusion. Steroids and thrombolysis are not routine in post-ROSC care unless indicated for other conditions. Air crews must coordinate with receiving centers, monitor for arrhythmias, and avoid overshoot hypothermia. TTM is a guideline-supported neuroprotective strategy during the post-arrest phase.
