Anterior STEMI (V1–V4 elevation) reflects LAD occlusion, which supplies the anterior wall and septum. RCA occlusion produces inferior changes (II, III, aVF). Circumflex typically produces lateral changes (I, aVL, V5–V6). Left main may cause diffuse ST depression with aVR elevation. LAD occlusion is associated with high morbidity due to large myocardial territory. Air crews should recognize this, administer aspirin, oxygen if hypoxemic, and expedite reperfusion activation with receiving centers.

Gases expand with falling ambient pressure (Boyle’s law). Complementary measures include nasogastric decompression when appropriate, avoiding excessive bagging that insufflates the stomach, and monitoring for emesis. In fixed-wing operations, pre-planned cabin altitude profiles for at-risk patients can prevent complications, improve ventilation mechanics, and enhance transport safety.

CPP = MAP – ICP. Here: 70 – 25 = 45 mmHg. Normal target CPP is 60–70 mmHg. A CPP of 45 is inadequate, risking ischemia. Interventions include sedation, head elevation, osmotic therapy, and ensuring normotension. In transport, continuous monitoring is rare, so crews must maintain MAP ≥70 and minimize ICP surges to preserve perfusion.

Norepinephrine is first-line for septic shock due to reliable vasoconstriction with limited tachycardia. Dopamine increases arrhythmias. Epinephrine is second-line or adjunct. Phenylephrine can reduce cardiac output by excessive vasoconstriction. In-flight, norepinephrine infusion via pump with careful titration supports perfusion while avoiding further harm. Rapid initiation of antibiotics and source control remain equally important.

Evidence shows higher success with fewer complications when decompression is performed at the 4th–5th intercostal space, anterior-to-mid axillary line, compared to the traditional 2nd ICS MCL, which often fails due to chest wall thickness. Technique: insert above the rib to avoid vessels. Securing catheter in the flight cabin is critical to avoid dislodgment. Definitive chest tube is needed as soon as feasible at destination.
