Lead II is commonly used for rhythm monitoring because it aligns with the heart’s electrical axis, producing tall P waves and clear QRS complexes, ideal for identifying atrial activity and arrhythmias. Lead I may be used in certain cases, but it provides less consistent P wave visibility. aVR is rarely used for monitoring alone. Right ventricular infarctions are best detected by right-sided precordial leads, labeled V1R–V6R. Among these, V4R is most sensitive. V7–V9 are posterior leads, useful for posterior infarction detection. Leads I and aVL view the lateral wall. V3 and V4 are anterior wall leads. Adding right-sided leads is especially important when inferior MI is suspected, as RV involvement affects management (e.g., preload sensitivity).

Holter monitoring provides continuous ECG data over 24–48 hours (sometimes longer). It is used to detect arrhythmias, correlate symptoms with rhythm, and evaluate ischemic changes during normal daily activities. It does not measure cholesterol or pulmonary function and cannot replace echocardiography, which assesses structure and function. Holter monitoring complements other diagnostic tests to give a fuller picture of cardiac status.
