Prognosis in ALL is favorable with modern risk-stratified therapy (chemotherapy ± CNS prophylaxis). Genetic risk factors (e.g., t(12;21) favorable, BCR-ABL unfavorable) guide management.

Deficiency of terminal complement components (C5–C9) leads to failure of membrane attack complex (MAC) formation. This predisposes to recurrent infections by Neisseria meningitidis and Neisseria gonorrhoeae, as the MAC is critical for bacterial lysis. C1q or C2 deficiencies predispose to immune complex disease and SLE-like syndromes. C3 deficiency causes recurrent pyogenic infections and severe encapsulated bacterial infections due to impaired opsonization. Recognition of C5–C9 deficiency is important in patients with recurrent meningococcal infections. Management includes vaccination against meningococcus and prophylactic antibiotics. This highlights the complement system’s role in innate immunity.

The trochlear nerve (cranial nerve IV) innervates the superior oblique muscle, which depresses and intorts the eye when adducted. Damage causes vertical diplopia, particularly when reading or descending stairs. Patients compensate by tilting the head away from the affected side, reducing diplopia. Oculomotor nerve palsy causes ptosis, dilated pupil, and eye deviation “down and out.” Abducens nerve palsy causes impaired abduction (horizontal diplopia). Facial nerve palsy causes ipsilateral facial muscle weakness, not diplopia. Trochlear nerve palsy is the only cranial nerve palsy where patients tilt the head away from the lesion to improve vision. Causes include trauma, microvascular ischemia, or congenital palsy. Management depends on etiology and may include prism glasses or surgical correction.

Renal cell carcinoma (RCC) commonly presents with the triad of hematuria, flank pain, and a palpable abdominal mass, though all three together are uncommon. RCC arises from proximal tubular epithelium and can invade the renal vein, producing varicocele (especially left-sided). A classic paraneoplastic feature is ectopic secretion of parathyroid hormone–related peptide (PTHrP), causing hypercalcemia. Additionally, RCC may secrete erythropoietin (leading to polycythemia), renin (causing hypertension), or ACTH. Hypocalcemia is not typical; calcitonin-producing tumors are more characteristic of medullary thyroid carcinoma. Insulinomas cause hypoglycemia but are pancreatic in origin. Management depends on stage—partial or radical nephrectomy for localized disease, targeted therapy (tyrosine kinase inhibitors, immunotherapy) for advanced disease. Surveillance for paraneoplastic syndromes is essential during follow-up.
